A medicine can work exactly as intended and still deserve a periodic review. The dose that made sense after surgery may remain on a list months later. A heartburn drug bought over the counter can become a daily habit. A new symptom may be treated with another prescription even when the first medication helped cause it.
Long-term effects vary by drug, dose, health history, and duration. They are not a reason to stop a prescription abruptly. They are a reason to know why each medicine is being used, what needs monitoring, and whether the expected benefit still outweighs the risk.
Proton Pump Inhibitors
Proton pump inhibitors, or PPIs, include omeprazole, esomeprazole, lansoprazole, and similar drugs. They reduce stomach acid and can heal ulcers, control severe reflux, treat erosive esophagitis, and protect people at high risk of gastrointestinal bleeding. Those are meaningful benefits.
Problems arise when a short over-the-counter course turns into indefinite use without reassessment. Long-term or high-dose therapy has been associated with low magnesium, vitamin B12 deficiency, certain intestinal infections, kidney problems, and osteoporosis-related fractures in some patients. The strength of evidence differs across these outcomes; several associations come from observational studies and do not prove that PPIs caused every event.
FDA-approved omeprazole labeling warns about C. difficile-associated diarrhea, low magnesium, and fracture risk with long-term multiple-daily dosing. Someone with Barrett’s esophagus, severe esophagitis, a history of bleeding ulcers, or continued use of certain anti-inflammatory drugs may have a strong reason to stay on a PPI. The review should focus on indication and dose rather than an automatic stop date.
Nonsteroidal Anti-Inflammatory Drugs
Ibuprofen and naproxen are familiar enough to feel harmless. They can reduce pain and inflammation, but regular use can irritate the stomach, cause ulcers or bleeding, reduce kidney function, raise blood pressure, worsen fluid retention, and increase cardiovascular risk in susceptible people.
Risk rises with age, higher doses, longer duration, kidney disease, heart failure, a history of ulcers, and use alongside blood thinners, corticosteroids, or other NSAIDs. Taking ibuprofen while a prescription anti-inflammatory is already on the medication list can unintentionally double the exposure.
The National Institute on Aging’s pain guidance cautions that long-term NSAID use can cause internal bleeding and kidney problems. A clinician may suggest the lowest effective dose, a topical option, acetaminophen for an appropriate type of pain, physical therapy, or a different strategy based on the cause.
Anticholinergic Medications
Anticholinergic effects appear in many drug classes, including some sleep aids, older antihistamines, bladder medications, antidepressants, and drugs used for nausea or dizziness. Common effects include dry mouth, constipation, blurred vision, urinary retention, confusion, and impaired balance.
The burden can accumulate when several mild anticholinergic drugs are taken together. An older adult might use diphenhydramine for sleep, a prescription for overactive bladder, and another medication with similar properties without realizing they overlap.
Observational research has linked heavier cumulative exposure to an increased risk of dementia. The National Institute on Aging’s summary of this research is careful about what the study can show: it found an association and could not establish that the drugs directly caused dementia. Cognitive changes, falls, constipation, and urinary problems remain immediate reasons to review the regimen.
Corticosteroids
Prednisone and other systemic corticosteroids can control severe inflammation, autoimmune disease, and dangerous allergic or respiratory conditions. With sustained exposure, they can also raise blood sugar and blood pressure, weaken bones, thin skin, affect mood and sleep, increase infection risk, contribute to cataracts or glaucoma, and suppress the adrenal glands.
The risks depend heavily on dose and route. A short course is different from months of daily tablets. An inhaled or topical steroid usually produces less whole-body exposure, although technique, potency, site, and duration still matter.
Systemic corticosteroids should not be stopped suddenly after prolonged use. The body may need time to resume normal cortisol production, so clinicians often taper the dose. Monitoring may include glucose, blood pressure, eye health, bone density, and strategies to protect bone depending on the person’s exposure and risk.
Acid-Suppressing, Sedating, and Blood-Pressure Effects Can Combine
Medication risk rarely lives in one bottle. A sedating pain medicine, a sleep aid, and an anxiety drug may each be tolerable alone but cause dangerous drowsiness together. Several drugs that lower blood pressure can contribute to dizziness when dehydration or illness is added. A diuretic, an ACE inhibitor, and an NSAID can place particular stress on kidney function.
Alcohol, cannabis, supplements, and nonprescription products belong in the same review. “Natural” does not mean inactive. Some supplements affect bleeding, blood pressure, glucose, liver enzymes, or the way prescription drugs are metabolized.
Antidepressants and Other Psychiatric Medicines
Antidepressants can be effective for depression, anxiety disorders, chronic pain, and other conditions. Long-term concerns differ across drug classes and may include sexual side effects, weight change, sweating, sleep disruption, emotional blunting, low sodium in susceptible older adults, and discontinuation symptoms.
A difficult withdrawal is not the same as addiction. The nervous system adapts to regular exposure, and stopping too quickly can cause dizziness, flu-like sensations, sleep disturbance, anxiety, or electric-shock sensations. A gradual plan designed with the prescriber is safer than skipping doses at random.
Benzodiazepines require separate care. Regular use can lead to physical dependence and tolerance, while sedation and impaired coordination can increase falls and driving risk. Combining them with opioids or alcohol can suppress breathing. A taper may need to proceed slowly, especially after long-term use.
Diabetes Medications Need Nutritional Context
Metformin has decades of clinical use and a strong safety record for many people with type 2 diabetes. Over time it can reduce vitamin B12 absorption in some patients. Tingling, anemia, fatigue, or balance changes may prompt testing, and periodic monitoring can be reasonable during long-term therapy.
Insulin and sulfonylureas can cause low blood sugar. Changes in appetite, kidney function, weight, alcohol intake, or meal timing can alter the dose a person needs. Newer glucose-lowering drugs have their own benefits and cautions. The right monitoring plan depends on the specific agent rather than the broad label “diabetes medicine.”
Statins and Muscle Symptoms
Statins reduce heart attacks and strokes in people whose baseline risk justifies treatment. Muscle aches occur in clinical practice, although symptoms are not always caused by the drug. Severe muscle injury is rare. Statins can also modestly increase blood sugar and the likelihood of diabetes in people already predisposed to it.
That small metabolic risk is usually outweighed by cardiovascular benefit in a high-risk patient. If muscle symptoms begin, the options include checking for other causes, reviewing interactions, adjusting the dose, trying a different statin, or using another lipid-lowering drug. Quietly abandoning treatment leaves the original cardiovascular risk unaddressed.
A Medication Review Should Answer Five Questions
- What condition is this treating?
- Is it helping in a way that can be observed or measured?
- Does the dose still fit current kidney function, liver function, age, and weight?
- What side effects, laboratory tests, or interactions need attention?
- What would a safe reduction or stop plan look like if it is no longer needed?
Bring every prescription, over-the-counter drug, vitamin, and supplement to the review. Include products used only occasionally. The National Institute on Aging recommends keeping a complete list and notes that polypharmacy increases the chance of side effects and unintended problems.
A review is particularly valuable after hospitalization, a new diagnosis, a change in kidney function, a fall, new confusion, a transition between clinicians, or the addition of several drugs. Pharmacists can identify duplicate ingredients and interactions that are easy to miss across different prescribers.
Do Not Turn a Review Into an Abrupt Stop
Some medications can produce dangerous rebound symptoms or withdrawal when stopped suddenly. Corticosteroids, benzodiazepines, certain antidepressants, seizure medicines, beta blockers, and clonidine are examples that may require a planned taper. The condition being treated can also return.
The useful habit is simple: attach an expiration date to the decision, not necessarily to the prescription. Ask at the next visit whether the reason, dose, and monitoring still make sense. Medicines deserve the same follow-up as the diseases they treat.
