A triglyceride result can improve substantially, yet the right response starts with the number and the reason behind it. A fasting value of 190 mg/dL raises a different concern from a result above 1,000 mg/dL. The first calls for a careful cardiovascular risk plan. The second can carry an immediate risk of pancreatitis and deserves prompt medical management.
Triglycerides are the body’s main storage form of fat. Levels rise after meals because the bloodstream carries energy toward tissues. Persistent elevation can reflect excess energy intake, insulin resistance, diabetes, alcohol use, certain medicines, kidney or thyroid disease, genetics, or a combination. Lowering the value works best when the plan addresses those drivers.
Start With the Actual Risk
A triglyceride level below 150 mg/dL is generally considered normal. Results from 150 to 499 mg/dL are often described as mild to moderate elevation, while 500 mg/dL or higher is severe. Very high levels, especially 1,000 mg/dL or above, increase concern for acute pancreatitis. Severe upper abdominal pain, persistent vomiting, fever, or pain radiating toward the back warrants urgent evaluation.
Triglycerides also contribute to the broader pattern of cardiovascular risk. The American Heart Association’s current lipid guidance advises interpreting them alongside LDL cholesterol, HDL cholesterol, diabetes, blood pressure, smoking, family history, and prior cardiovascular disease. A single target cannot replace that full assessment.
If a result was unexpected, confirm whether the test was fasting and review recent illness, alcohol intake, blood glucose, thyroid and kidney function, pregnancy status when relevant, and medications. Estrogens, steroids, some antipsychotics, certain HIV treatments, and other drugs can contribute. Keep taking prescribed medicine unless the prescribing clinician changes it.
Food Changes With the Largest Payoff
Triglycerides respond strongly to the balance between energy coming in and energy being used. The most productive diet changes usually focus on added sugar, refined starch, alcohol, food quality, and portions rather than one branded eating plan.
Reduce Added Sugar and Refined Carbohydrates
Sugary drinks, sweets, and large portions of refined grains can deliver carbohydrate quickly, especially when total energy intake already exceeds need. The liver can convert some of that surplus into triglycerides. Replace sweetened drinks with water or unsweetened options. Build meals around vegetables, legumes, intact whole grains, and protein sources that slow the pace of digestion.
Fruit does not need to disappear from a heart-healthy diet. Whole fruit brings fiber and requires more chewing than juice. Large servings of juice, sweetened coffee drinks, and soda are easier places to find concentrated sugar that adds little fullness.
Treat Alcohol as a Major Variable
Alcohol can raise triglycerides considerably in susceptible people. When levels are severely elevated, clinicians commonly advise avoiding it. For lower elevations, reducing frequency and amount may produce a meaningful change. The American Heart Association’s 2026 dietary statement advises people who do not drink to avoid starting and people who drink to limit intake.
Choose a Sustainable Fat Pattern
Use unsaturated fats from foods such as nuts, seeds, olive or other non-tropical plant oils, and fish in place of saturated fat from butter, fatty meats, and some highly processed foods. Fat still contains substantial energy, so portions matter when excess weight is contributing. Omega-3-rich fish can fit this pattern. High-dose prescription omega-3 products are different from ordinary fish-oil supplements and should be used only through a clinician-directed plan.
Movement and Weight Change Work Together
Regular aerobic activity improves the body’s use of circulating fuel and can lower triglycerides. Aim toward at least 150 minutes of moderate-intensity activity each week, with muscle-strengthening work on two days, if your health and mobility allow. Start below that level when needed. Ten-minute walks after meals, a consistent cycling routine, or swimming can build useful weekly volume.
For people carrying excess body fat, modest weight loss can improve triglycerides, glucose control, blood pressure, and liver fat. The process should protect muscle through adequate protein and resistance training. A steady plan that can be continued usually produces more durable metabolic gains than repeated severe restriction.
Sleep, smoking cessation, and diabetes management belong in the same plan. Short sleep can make appetite and glucose regulation harder. Smoking adds vascular risk even when triglycerides improve. Persistently high glucose can continue pushing triglyceride production until diabetes treatment is addressed.
When Medication Enters the Discussion
The 2026 American Heart Association dyslipidemia guideline summary states that statin therapy remains the foundation of drug treatment for persistently elevated triglycerides when the aim is reducing atherosclerotic cardiovascular risk. For triglycerides at or above 1,000 mg/dL, triglyceride-lowering therapies may also be used to reduce pancreatitis risk.
That distinction matters. A medicine selected to reduce heart attack and stroke risk may differ from a therapy used because triglycerides have reached a pancreatitis range. Prescription omega-3 fatty acids, fibrates, statins, and other drugs have different effects, interactions, and evidence. Over-the-counter supplements are not interchangeable with prescription products.

Choose two or three changes that match the likely cause: remove sugary drinks, pause alcohol, walk after dinner, improve diabetes control, or prepare more meals from minimally processed foods. A repeat lipid panel after the interval recommended by your clinician shows whether the plan is working.
Bring the result, medication list, and a brief record of the changes you actually made. That evidence helps separate an ineffective strategy from one that simply needs more time. The useful outcome extends beyond a lower laboratory number. It is a lower risk of pancreatitis when levels are severe and a stronger overall cardiovascular risk profile for the years ahead.
